How to send in orders:

1. Have new members fill out a Membership Record Form signed by the chapter officers and the chapter advisor.

2. Create a spreadsheet file (Microsoft Excel or Quattro Pro) with the following fields:

   First Name

   Middle Name

   Last Name

   Street Address to send BioScience

   City

   State

   Zip

   e-mail

3. Send an e-mail to C.J. Wong at phisigmasociety@hotmail.com with the spreadsheet file attached.   In the text of the e-mail be sure to indicate your chapter name, address, and the date of your initiation.

4. Mail the membership record forms (paper copies), along with the Official Annual Report Form to Catherine Wong, 405 South Elm St., Wallingford, CT, 06492 (effective August 15, 2001).

5. If you are unable to submit your spreadsheet file electronically, please copy it onto a floppy disk and send via snail mail to Catherine Wong.  On the floppy disk, please indicate your chapter name, address, and date of your initiation.

Please Note:

• Photocopy a set of records for your chapter files!

• Include one check or money order for the entire initiation class.  Indicate chapter name on the check or money order.   The check can be sent along with the membership record forms (paper copies) to Catherine Wong, 6 Hawthorne Street, Beverly, MA  01915.

• Allow 6-8 weeks for printing of certificates and mailing them to you.

Phi Sigma Advisors-- We need your help!

• Where would you like to see a Phi Sigma Chapter?

• Who do you know there that would make a good advisor?

• Would you be willing to visit potential colleges/universities in your area?

Phi Sigma Advisors-- The National Office would like to add a Chapter Advisor Listing to the National WebPage, including a description of each chapter advisor and a photo. 

If you would like to be included, please e-mail us a description of your teaching, research, and service activities.  Please limit your description to a maximum of 75 words.  If you would like a photo included, please send it as an e-mail attachment to C.J. Wong at: phisigmasociety@hotmail.com.

Advisors -- Please inform your students about the travel grant awards!!

Advisors- Please check the Active Chapters List at the top of this page to see if your contact information is correct.  If any information is incorrect, please e-mail C.J. Wong at phisigmasociety@hotmail.com.

Advisors -- What do you think of the new webpage?  Please send corrections, the URL of your chapter's webpage, and any other links you would like to see added (employment, grad school, publishing, fund raising, etc.).
